Express Scripts Tightens Commercial Formulary Control With “Not Covered” List

Pharmacy benefit manager Express Scripts Holding Co. is stepping up formulary controls in its commercial plan business by introducing a list of drugs that are excluded from coverage beginning Jan. 1, 2014.
